There are many licensing options. Some clients prefer to pay monthly. Others pay outright and then just pay for support. Others pay for an annual license. 

I'd rate the solution three out of five in terms of affordability.

There are two or three different models, but you can actually have a CapEx where you purchase the unit outright and then there's a yearly maintenance cost on top for years two and three, and four, or you can actually do a subscription purchase through Citrix cloud and you pay on a monthly basis, or you can have a three-year subscription contract as well. I give the solution a six out of ten for its cost. The solution is not cheap but it's not overly expensive.

Compared to other products, WAF is more costlier than LB, but in Citrix, LB is more expensive than WAF. We have a yearly subscription. You may get a discount if you go for three years. There is an additional cost for support, and the license covers only VM.

As mentioned before, setup is fairly simple and documented on the Citrix eDocs. 
there's a lot of information and how-to articles which facilitates all the setup.

The new pooled licensing model also can be very useful in hybrid or wide deployments.

Apart from licensing, there are additional fees for support and partnerships.
